Method: The data used in these heat maps were collected manually by ACR staff through state web sites containing individual Medicaid physician fee schedules and in some instances through direct outreach to Medicaid administrators’ offices. Data were initially acquired at the end of 2012 and again in the last quarter of 2022. Medicare data are publicly available on their Physician Fee Schedule page.
